Copay Program Terms and Conditions

The Nuvalent Patient Support Copay Program (“Copay Program” or “Program”) provides financial assistance for qualified, commercially insured patients who have been prescribed a Nuvalent medication. Patients must meet all eligibility requirements to participate in the Copay Program and are subject to the financial limits and the Program’s scope as presented in the Terms and Conditions. If eligible for the Copay Program, patients may pay as little as $0 for their prescribed Nuvalent treatment, each time it is dispensed.

To be eligible for the Copay Program patients must:
  1. Be at least 18 years of age.
  2. Reside in the US, including Puerto Rico and other US territories.
  3. Have received a prescription for a Nuvalent medication for an indication approved by the Food and Drug Administration (FDA).
  4. Be commercially insured and covered for the prescribed medication.
Patients are not eligible for the Copay Program if:
  1. The patient is insured through a government- or state-sponsored program (including, but not limited to Medicare, Medicare Advantage, Medicaid, TRICARE/Department of Defense, Veterans Affairs).
  2. The patient’s insurance has prohibited the use of the Copay Program.
  3. The patient self-pays for their prescription without insurance.

This copay offering is limited to 1 (one) enrollment per person. Once enrolled, there is a maximum benefit per calendar year; after the maximum benefit has been reached, the patient assumes financial responsibility for copay costs. This benefit resets on January 1 each year that the patient remains enrolled and eligible. The Program will cover the copay costs associated with a Nuvalent medication. It does not cover any other healthcare provider charges or any other treatment costs. This offer may not be combined with any other coupon, free trial, discount, prescription savings card, or offer.

Patients must consent to data sharing related to Program administration to enroll in the Copay Program. Knowingly providing false or misleading information can lead to termination of use of the Program. Data related to a patient’s participation in the Program may be collected and shared with Nuvalent and the Copay Program vendor. The data may be analyzed for purposes related to assessing and operating the Copay Program and to verify eligibility for the Program.

Patients must also attest to having reviewed program eligibility requirements, having no government- or state-sponsored insurance, and consent to data collection, use, and sharing as described in these Terms and Conditions. Patients must notify the Program promptly if they become ineligible to participate in the Copay Program or their insurance coverage changes, including if they become enrolled in any government- or state-sponsored insurance program. If a healthcare provider is enrolling on behalf of a patient, the healthcare provider must attest to having reviewed program eligibility requirements with the patient, confirm that the patient does not have government- or state-sponsored insurance, and have obtained consent from the patient for data collection, use, and sharing as described in these Terms and Conditions.

No purchase is necessary to participate in the Copay Program. The Program is not health insurance. Participation in the Copay Program is void in regions in which it is prohibited by state law, taxed, or restricted. The copay offering cannot be sold, purchased, traded, or counterfeited. Nuvalent reserves the right to modify or terminate the Copay Program offerings at any time, without notice.
