Copay Assistance Program*
Eligible commercially insured patients prescribed JIDEYTRO may pay as little as $0. Terms and conditions apply.*
Key eligibility criteria:
- Commercial insurance for prescription coverage
- Have a valid prescription for an approved indication for JIDEYTRO from a licensed US healthcare provider
- Reside in the US, including Puerto Rico or other US territories
Enrollment in Nuvalent Patient Support is not required.

Patient Assistance Program (PAP)†
The Patient Assistance Program provides JIDEYTRO at no cost to eligible patients who are uninsured, underinsured, or unable to afford their medication. Eligibility criteria are detailed below. Program availability and terms and conditions apply.†
Key eligibility criteria:
- Patient is uninsured, underinsured, or under financial strain, even with insurance coverage
- If patient’s insurance does not cover JIDEYTRO, provide a copy of the first-level appeal denial letter with the PAP application. The denial must be for a payer-related reason
- Be at least 18 years of age
- Have a valid prescription for JIDEYTRO from a licensed US healthcare provider
- Reside in the US, including Puerto Rico or other US territories
- Be enrolled in Nuvalent Patient Support
- Income limits apply

Nuvalent Patient SupportEnrollment
Quick Start Program†
The Quick Start Program provides eligible patients who are newly prescribed JIDEYTRO and are experiencing a coverage or insurance delay with a temporary supply at no cost. Program availability and terms and conditions apply.†
Key eligibility criteria:
- New to therapy
- Have commercial and/or government insurance with a payer-related delay in coverage determination
- Be at least 18 years of age
- Have a valid prescription for an approved indication for JIDEYTRO from a licensed US healthcare provider
- Reside in the US, including Puerto Rico or other US territories
- Be enrolled in Nuvalent Patient Support

Nuvalent Patient SupportEnrollment
Bridge Program†
The Bridge Program provides eligible patients who are already on JIDEYTRO and are experiencing a coverage gap or delay with a temporary supply of JIDEYTRO at no cost. Program availability and terms and conditions apply.†
Key eligibility criteria:
- Actively being treated with JIDEYTRO
- Have a short-term gap in commercial insurance coverage
- Be at least 18 years of age
- Have a valid prescription for an approved indication for JIDEYTRO from a licensed US healthcare provider
- Reside in the US, including Puerto Rico or other US territories
- Be enrolled in Nuvalent Patient Support
