Quick Start Program Terms and Conditions

The Nuvalent Patient Support Quick Start Program (“Quick Start Program” or “Program”) provides two 30-day fills of the medication at no cost to eligible patients who have been waiting at least 3 days for a decision related to insurance approval for their prescribed Nuvalent medication. The second fill will be dispensed if the patient still qualifies.

To participate in the Quick Start Program, patients must:
  1. Be at least 18 years of age.
  2. Reside in the US, including Puerto Rico or other US territories.
  3. Have a valid prescription for the Nuvalent medication from a licensed US healthcare provider.
  4. Be new to treatment with a Nuvalent medication for an indication approved by the US Food and Drug Administration (FDA).
  5. Be enrolled in Nuvalent Patient Support with a completed and signed Patient Authorization.
  6. Have commercial and/or government or state-sponsored insurance.
  7. Have an actively pending prior authorization (PA) delay. Patients agree to notify the Program of any coverage changes and to provide information requested for eligibility verification.

This benefit is non-transferable, cannot be sold or shared, and may not be combined with any other coupon, free trial, discount, prescription savings card, or offer. There are no income limits for enrollment in the Quick Start Program.

No purchase is necessary to participate in the Quick Start Program. The Program is not health insurance, and participation is not a guarantee of coverage. Participation in the Quick Start Program is void outside of the US and US territories and in regions in which it is prohibited by state law, taxed, or restricted. Medication provided through this Program is provided at no charge and cannot be sold, purchased, billed, or traded, or submitted to any payer for reimbursement. Additional eligibility requirements and Program terms and conditions may apply. Nuvalent has sole discretion to determine eligibility for the Program and reserves the right to modify or terminate the Quick Start Program offerings at any time, without notice.

Nuvalent Patient Support Bridge Program Terms and Conditions

The Nuvalent Patient Support Bridge Program (“Bridge Program” or “Program”) provides two 30-day fills of the medication at no cost to eligible patients with a temporary gap in insurance coverage. The second fill will be dispensed if the patient still qualifies.

To participate in the Bridge Program, patients must:
  1. Be at least 18 years of age.
  2. Reside in the US, including Puerto Rico or other US territories.
  3. Have a valid prescription for the Nuvalent medication from a licensed U.S. healthcare provider.
  4. Be an existing patient receiving treatment with a Nuvalent medication for an indication approved by the US Food and Drug Administration (FDA).
  5. Be enrolled in Nuvalent Patient Support with a completed and signed Patient Authorization.
  6. Have or had commercial insurance; and
  7. Be experiencing a temporary gap in insurance coverage. Patients agree to notify the Program of any coverage changes and to provide information requested for eligibility verification.

This benefit is non-transferable, cannot be sold or shared, may not be combined with any other coupon, free trial, discount, prescription savings card, or offer and is not contingent on any past or prior purchase. There are no income limits for enrollment in the Bridge Program.

Patient Assistance Program (PAP) Terms and Conditions

The Nuvalent Patient Support PAP (“PAP” or “Program”) provides medication at no cost to eligible patients who cannot afford their medication.

To participate in the PAP, patients must:
  1. Be at least 18 years of age.
  2. Reside in the US, including Puerto Rico and other US territories.
  3. Have a valid prescription for the Nuvalent medication from a licensed US healthcare provider.
  4. Be enrolled in Nuvalent Patient Support with a completed and signed Patient Authorization; and
  5. Submit a denial letter with a valid payer-related reason for denial of coverage of their prescribed Nuvalent medication with an appeal exhausted OR have no insurance and be unable to afford the medication OR have government insurance with a patient copay and be unable to afford the medication. Income limits apply.

To participate in the PAP, the patient must complete and submit the Nuvalent Patient Support Enrollment Form and consent to validation of their income by Nuvalent or provide qualified proof of income documentation. Nuvalent will periodically reach out to confirm eligibility; the patient must respond to remain eligible for the PAP. Patients must not count free product received from the PAP as an expense incurred for purposes of determining out-of-pocket costs for any plan, including true out-of-pocket costs (“TrOOP”) for purposes of calculating the out-of-pocket threshold for Medicare Part D plans.

Eligible patients enrolled in the PAP receive their prescribed Nuvalent medication every 30 days, as long as they remain eligible and will be screened periodically. Patients who are a member of a Medicare Part D Plan who meet the PAP eligibility criteria are eligible to receive the Nuvalent medication for the entire coverage year, and Nuvalent will notify your Medicare Prescription Drug Plan, if applicable, that you are receiving the medication outside of the Medicare Part D benefit. Patient eligibility must be confirmed annually for continued enrollment. This offer may not be combined with any other coupon, free trial, discount, prescription savings card, or offer.
